I am trying to get Medicare Part B now that I am retired. Medicare has a form CMS L564 that is asking for the date my insurance coverage with Walmart began and the date that it ended. How can I go about obtaining this information. I requested a work data sheet from the work number and I received it however it does not have the insurance info on it that i need.
